If you need to have equipment INSTALLED, DISCONNECTED, MOVED or CHANGED:

Be sure to include:

  1. What you want done.
  2. Location Information (please include BUILDING and ROOM).
  3. When you want it done by.
    (Note: requests are normally assigned a due date of 3 weeks from the received date. If you need work done sooner there may be an expedite charge, please call 7-6944)
  4. Billing Information (POETS or Billing Address)
  5. Your Contact Information
